🧫 Xavier Mulet @xavi1239 presented his work at #ES 🧫 Xavier Mulet @xavi1239 presented his work at #ESCMIDGlobal on an innovative AI-based approach for the rapid detection of high-risk Pseudomonas aeruginosa clones using MALDI-TOF spectra.

Using a large collection of clinical isolates, the study developed a predictive model capable of accurately identifying major MDR/XDR high-risk clones such as ST175, ST235, ST111 and the emerging ST773.

This approach provides a fast and powerful tool to support infection control and improve clinical decision-making, enabling more targeted and timely therapeutic strategies.

The study shows that, under β-lactam pressure, resistant subpopulations can emerge with increased MICs and cross-resistance to multiple antibiotics, including cephalosporins and carbapenems. Notably, these changes were not driven by mutations in TEM-1 or TEM-135 β-lactamases, but likely by chromosomal adaptations and other regulatory mechanisms.

These findings highlight the complexity of resistance evolution and reinforce the need for genomic surveillance and alternative therapeutic strategies against N. gonorrhoeae.

Analysing data from 2008 to 2025, the results show a clear improvement in carbapenemase detection over time, with participating centres increasingly able to correctly identify these resistance mechanisms.

This progress likely reflects advances in diagnostic tools—such as automated systems, immunochromatography and PCR—as well as continuous training efforts supported by the SEIMC Quality Control Program.

Her study shows that resistance evolution is shaped by both the initial bacterial genotype and the treatment regimen. While dual therapy constrains the maximum resistance levels to β-lactams, it can also select for hypermutator strains, influencing adaptation pathways.

These findings highlight the importance of genotype-guided therapeutic strategies to better preserve antibiotic efficacy against N. gonorrhoeae.

His work identifies the V516M mutation in PBP2 as a key driver of zidebactam resistance, significantly reducing binding affinity and altering susceptibility profiles. Importantly, cross-resistance patterns were shown to depend on PBP-binding interactions, impacting some antibiotics while leaving others unaffected.

These findings highlight the complexity of antimicrobial resistance and emphasize the importance of detailed PBP-binding profiling to guide future therapeutic strategies.
